Comparison of Mesh Fixation Method in Inguinal Hernia Repair with PHS

Abstract
Purpose: Tension-free repair is the most commonly used technique for inguinal hernia repair. Tension-free method enables to reduce the recurrence rate and pain. Sutures and staples are used to fix mesh. Fibrin glue is a new method to fix the mesh, which is expected to reduce the postoperative complication rate.

Methods: We made a retrospective study of 280 patients that were operated by a single surgeon at the Gil Hospital, Gachon University of Medicine and Science. We analyzed 280 cases of inguinal hernia patients that underwent repair with PHS from August 2007 to August 2009. We compared postoperative complications of 140 patients of the fibrin group and 140 patients of the suture group.

Results: Complications of the group that used fibrin glue were as follows: hematoma in 18 cases, seroma in 3 cases, and wound swelling in 2 cases. Complications of the suture group were as follows: hematoma in 8 cases, seroma in 3 cases, and testicular swelling in 3 cases. Hematomas were the most common complication in both groups.

Conclusion: Fibrin glue is used to fix the mesh in hernias with mesh. Other studies have reported that using fibrin glue at hernia operation reduces some complications. Bur in our study, there was no difference of complications between the fibrin glue group and the suture group.
